Wedding Day Questionnaire Bride's Name: First Name Last Name Bride's Phone Number: (###) ### #### Bride's Email Address: Groom's Name: First Name Last Name Groom's Phone Number: (###) ### #### Groom's Email Address: Wedding Date MM DD YYYY Will you have a wedding planner or person in charge the day of your wedding? If yes what is their name and phone number? Ceremony location and address: Reception location and address (if different from ceremony): You have us booked for an 8 hour wedding package. When would you like us to arrive and leave? (This can also be narrowed down after we go through this questionnaire, this just helps us know what your thoughts are.) How many guests will be at your wedding/reception?: Bride starts getting ready at (time and place): Groom starts getting ready at (time and place): We really enjoy the moment the bride puts on her dress, with help of mom or someone close to you. While many brides want photographs of this, we do understand that some prefer it to be more private. Would you like me (Chantell) to be there while you put your dress on? (keep in mind all photos taken will be tasteful) *Please note that anyone helping with putting your dress on likely will be in photos. Some moms or siblings would want to have their dress on and/or make up done by this time but this is completely up to you/their preference. What time (approximately) will you be putting your dress on? Are there any other morning rituals/things you are planning with your girls/guys seperately in the morning before your first look that you would like photographed? (girls in robes, guys at breakfast, gifts to bridal party, etc) We also strongly suggest doing a "First Look", where the bride and groom see each other for the first time. This allows for an emotional meeting of one another and is a good lead into bride and groom photos. Would you like us to capture your "First Look"? Do you have a location in mind for the "First Look"? (Brett and I are also awesome at picking out spots if you don't have ideas :) It is usually best for the first look location to be at the same spot as the start of bride/groom photos.) Time of first look: Will there be any exchange of notes/letters/gifts between bride and groom that you would like captured in your photos upon receiving/giving? If so, when will these be given? (prior to seeing each other, at first look, after ceremony) Wedding party photos start at (should be approx 1 hour after first look) Names of ALL wedding party members (bridesmaids, personal attendants, groomsmen, ushers, flower girls, ring bearers) Would you like individual photos with each of your bridesmaids/groomsmen? If having flower girl(s) and ring bearer(s), would you prefer they come at family photo time instead of wedding party photos? (This is sometimes helpful is they are young kiddos!) If having ushers and personal attendants, will they be present for bridal party photos? If yes, would you like them in one of the full bridal party photos or just a photo with groom or bride/groom? Family photos before or after ceremony? (BEFORE ceremony is usually the best time but we understand if there are some members that cannot come before - we can arrange to do a few photos at social hour/reception time if needed. We usually plan for a location as close to or at the ceremony site as possible. ) Any specific family situations we should know about/plan to accommodate for? (divorce, death, family members not speaking to one another) Are there any rules pertaining to photography that we should be aware of during your ceremony? (no flash? no walking past a certain aisle?) Please give me copies of any rule sheets given to you by the venue and also list them below Is there anything happening customary or unique during the ceremony that will be special to capture and helpful for us to have prior knowledge of? Please list general order of events during ceremony if known at this point. Ceremony starts at Ceremony ends at Are you planning on doing a receiving line or dismissal after your ceremony? (Please note this usually takes about 15 minutes per 100 guests.) Will there be a cocktail/hangout/social hour following your ceremony? Where will this take place? If so, what time will this begin and end? Reception starts at Will the bride & groom & bridal party be introduced in a special way? Will you have assigned seating at your reception? If so, please seat us in an area where it won't take too many hops and skips over chairs to get shots of kisses or speeches, etc. We DO NOT need to be right up front with your family but somewhere near the center and a few tables back usually works great. Is there anything happening at the reception that would be unique or helpful for us to have prior knowledge of? Speeches start at (approx) How many speeches do you have planned and who will be giving them? Do you plan on having a cake cutting? If so, when will this be done? (before/after dinner?) Sometimes the most relaxed photos between the bride and the groom are taken post-ceremony. As it is nice to take most photos prior to ceremony (as long as seeing each other before this), we often like to "steal the couple away" at some point either after dinner or before/shortly after dance starts to snap a few photos, especially if the light outside is "golden". (In the winter, this is often something we like to do at the end of social hour depending on timing.) Is this something you would be interested in? First dance starts at (if having a dance) What "special dances" will be taking place at the start of the dance? (first dance, father/daughter, mother/son, etc) We usually do not plan to stay for an entire wedding dance. Generally, we usually like to allow 30-45 minutes of dance coverage. After this, everything starts to look about the same :) Is there a special exit the bride and groom will be having that you would want to be photographed? (sparkler send off, lantern lighting, friend/family tunnel) Please keep this time in mind with the hours included for coverage in your package. We can always discuss adding on hours if this is something you would like to be covered. Wedding coordinator (name/website) Videographer (name/website/email) Hair and make-up (salon or company name) Wedding dress designer (and accessories if applicable) Bridesmaid dress designer Groom's suit/tux designer Groomsmen suit/tux designer Wedding officiant name Florist (name/website) Caterer (name/website) Cake Maker (name/website) Band/DJ (name/website) With all being said and done, do you still feel comfortable with the package you have chosen? FINALLY, please take the following space to let me know your "must-haves" or the most important areas of your wedding day that would be most special to you to have fully captured through my photography. I promise to deliver the best coverage I can throughout your day but it is helpful to be prepared ahead of time regarding your utmost anticipated moments you want photographed. (examples could be: getting my dress on, moments with mom, first look, bride and groom portraits, ring shots, candid moments of bridal party, unity ceremony, guest shots, first dance, etc) I will plan to build in more time for the moments you want and/or put extra focus on these things. Thank you!
